EAST LANSING, Mich. Often faced with overwhelming anxiety, patients newly diagnosed with lung cancer can find themselves in distress, and new research recommends nurses play a key role in alleviating concerns, leading to a better quality of life for patients.

A diagnosis of lung cancer the leading cause of cancer death in the United States brings with it high levels of stress and raises existential issues and death-related thoughts and concerns in patients, said Rebecca H. Lehto, assistant professor in the College of Nursing at Michigan State University.

In a paper published in the current edition of the Clinical Journal of Oncology Nursing, Lehto lays out several ways nurses can help bridge the gap between diagnosis and a strong psychological outlook during treatment.

"People facing life-threatening diagnoses often feel alone, and newly diagnosed lung cancer patients with early stage disease can have concerns about an uncertain future, the potential for treatment failure, the cancer spreading and the possibility of death," she said. "Health care providers, who are uncomfortable with and avoid discussing end-of-life questions, may contribute to a patient's alienation.

"Nurses who are comfortable with listening for and discussing existentially related concerns may be in a better position to promote the patient's psychological adaptation."

Too often, health care providers are solely focused on treatment and disease management, Lehto said. Her research identifies the types of death-related concerns that people experience and strategies to help resolve the normal but often distressing psychological responses in the early post-diagnostic and treatment period.

"With a nurse's help, if a patient can resolve the issues that impact quality of life during early treatment, it may greatly improve their outlook," she said. "It is difficult for patients to focus on learning about anti-cancer treatment if they are overwhelmed with anxiety and distressing thoughts."

Lehto said the biggest hurdle to overcome often is helping patients come to grips with the impact of the disease and the possibility of death on loved ones.

"Nurses first of all need to be a supportive, nonjudgmental presence," she said. "Patients are making life decisions and sometimes are making death preparations. Nurses can provide patient education materials and offer support in managing life affairs."

Other strategies include exploring life stories, discussing personal relationships and spiritual resources and helping patients locate resources to make funeral arrangements, update wills and other legal issues. Nurses can listen for patient concerns and focus only on personally relevant information so as not to further overwhelm them.

Lehto said the nurse role is essential despite concern that such discussions will increase psychological distress for patients.

"By addressing death-related concerns early, nurses will determine individualized needs for mental health or other supportive interventions or services," she said.

Lamar coach Pat Knight evokes dad in rant

Beaumont, Texas ? Lamar coach Pat Knight has torn into his players in a way that might remind some of his father.

On Wednesday night, after his team lost 62-52 to Stephen F. Austin, the younger and usually more subdued Knight pulled no punches in an extended rant that prompted comparisons to Bob Knight, his dad and a legendary coach known for his fiery temper.

"We?ve got the worst group of seniors right now that I?ve ever been associated with," Pat Knight said. "Their mentality is awful. Their attitude is awful. It?s been their [custom] for the last three years. To come out in a game like this, with no emotion and flat, it?s terrible."

Knight is in his first year at Lamar, a Southland Conference team, after being let go by Texas Tech. The Cardinals are 17-11 with three league games remaining. He said several times during Wednesday?s postgame news conference that he?s at the school to "clean things up" and that next year things will be different when the current seniors are gone.

"These guys are stealing money by being on scholarship with their approach to things," he said.

He said no one has stepped up to help lead the team in a better direction and says there?s an "infestation" of players who are difficult to coach.

"When I played, if you acted the way some of these guys did, you got shoved in a locker with a forearm up against your neck and told: ?You don?t do that. That?s not how we do things at Indiana,? and that?s what we need," Knight said while not specifying who did that when he was playing for the Hoosiers under his father.

After he called his seniors "the worst," he went down the box score and read off several players? statistics and said that they have an "attitude problem."

After that, he got much more personal.

"We?ve had problems with these guys off the court, on the court, classroom, drugs ...," Knight said. "If you act this way in the real world, you?re going to be homeless, without a job."

Knight?s rant begans when senior Mike James was at the podium beginning to answer a question. The coach stalked up to the podium while telling James to leave.

"All right, let?s go, I?m next," Knight said. "Because you don?t have a clue what it takes to win."

Messages left with a Lamar spokesman seeking comment on Knight?s rant weren?t immediately returned Friday afternoon.

The Cardinals clinched a spot in the Southland Conference tournament for the first time since the 2007-08 season on Feb. 15. But Knight believes they?ve been slacking off since then, and is upset in the way they?ve lost their last two games.

"You just can?t do all that BS and then expect to be a good team and win games," he said. "And if people have a problem with me being so harsh about it, I don?t care. I?ve come here to clean something up, and that?s the bottom line. We?ve got to get a better personality on this team, a better mindset, and that?s where recruiting comes into place."

He feels like they?re content to simply make the tournament and said that being their "monumental achievement" is sad.

Scores apply to test menus for Mars

Want to get paid $5,500, plus expenses,?for tasting different kinds of space food on Hawaii's Big Island for four months? Join the crowd: About 100 people have applied for the job so far, and there are still six days to go before the deadline.

There are a few catches, though: You'd have to be cooped up in a fake Mars habitat for most of that time, cut off from the rest of the world except for a time-delayed communication link. Forget about packing the bikini. Anytime you leave the habitat, you'd have to wear a bulky spacesuit. And don't expect a luau. The whole point of this exercise is to find out whether it's better to feed you freeze-dried and dehydrated foods, or let you make your own meals from "shelf-stable" ingredients such as flour, beans, rice and cheese. For 120 days, you'll have to write detailed assessments of all those meals ... as well as your own mood.

Such is the life awaiting six prime crew members and two alternates next year during a 120-day simulated Mars mission known as Hawaii Space Exploration Analogue and Simulation, or HI-SEAS.


University of Hawaii researcher Kim Binsted says the applications have been streaming in as the Feb. 29 deadline approaches. She can tell where HI-SEAS has gotten a shot of publicity by keeping track of where the emails are coming from on any particular day. "Apparently Italy heard about us yesterday," she said. By next week, she expects to have 200 or so applications to choose from.

The selection criteria are relatively stringent: a bachelor's degree in science, math or engineering ... three years of graduate school or professional experience ... ability to pass a flight physical exam ... 24 months of being tobacco-free. Plus, of course, a normal sense of taste and smell. "We're looking for people who would be as astronaut-like as possible," Binsted told me.

The simulation, conducted by researchers at Cornell University and the University of Hawaii at Manoa, is designed to find out which kinds of foods would make the most sense for a months-long mission beyond Earth orbit. Are meals be more satisfying if they're made from bulk ingredients, or will it turn out that the usual pre-packaged, no-muss meals are actually more suited to space missions?

After selection and training, the crew members will travel to Hawaii in early 2013 and get settled inside a simulated habitat that will probably be set up in the Big Island's Saddle Road area. Binsted said "it's very stark, very Marslike," with fresh lava flows from the Mauna Loa volcano. In addition to their food-tasting duties, the crew members should have some spare time to conduct other research studies, as long as they stay in character for the simulation.

Binsted said the entire three-year project is supported by a $947,000 NASA grant, and about a third of that will go toward the 120-day taste test. The project also includes a head-down, bed-rest study that's being conducted at the NASA Flight Analogs Research Center in Galveston, Texas, to simulate the effects of long-term microgravity.

Many astronauts have observed that food seems to lose its flavor in space ? which is why hot sauce is such a popular condiment for crews on the International Space Station. Binsted said the bed-rest study could determine whether the hot-sauce effect arises because of a physical effect (for example, swelling of the nasal passages in zero-G) or a psychological effect.

"There's not a lot of 'spice' in their life, so maybe they have to get it from their food," she said.

Lesbian federal worker wins health benefits case

(AP) ? A federal judge in San Francisco ruled Wednesday that the U.S. government cannot deny health benefits to the wife of a lesbian court employee by relying on the 1996 law that bars government recognition of same-sex unions.

U.S. District Judge Jeffrey White said that because the Defense of Marriage Act unconstitutionally discriminates against same-sex married couples, the government's refusal to furnish health insurance to Karen Golinski's wife is unjustified.

"The Court finds that DOMA, as applied to Ms. Golinski, violates her right to equal protection of the law ... by, without substantial justification or rational basis, refusing to recognize her lawful marriage to prevent provision of health insurance coverage to her spouse," White wrote in a 43-page decision that marks the third time in less than two years a federal court has declared the act unconstitutional.

Golinski, a staff lawyer for the 9th U.S. Circuit Court of Appeals, has been trying to secure spousal benefits for her wife, Amy Cunninghis, since shortly after the couple got married during the brief window in 2008 when same-sex marriages were legal in California. Her boss, Chief Judge Alex Kozinski, approved her request, but the Office of Personnel Management ordered Golinski's insurer not to process her application.

After Golinski sued, the Department of Justice originally opposed her in court but changed course last year after President Barack Obama and Attorney General Eric Holder said they would no longer defend the Defense of Marriage Act.

When White heard the case in December, the head of the Justice Department's civil division, Tony West, joined her lawyers from the gay rights legal group Lambda Legal in arguing on Golinski's behalf, leaving the job of defending DOMA to a lawyer hired by a House of Representatives group. The lawyers representing the Bipartisan Legal Advisory Group convened by House Speaker John Boehner did not immediately respond to an email to their offices sent after business hours Wednesday.

Former speaker and Minority Leader Nancy Pelosi issued a statement saying White's ruling demonstrated "that the House is not united in this case, that the BLAG lawyers do not speak for Congress, and that BLAG's intervention remains a waste of taxpayer resources."

Wednesday's ruling is the latest in an unbroken string of judicial setbacks for the Defense of Marriage Act, which Congress approved when states first started considering allowing gay and lesbian couples to get married. The law defines marriage as a union between a man and woman, and prohibits the government from granting benefits such as Social Security and Medicaid to same-sex couples.

A federal judge in Massachusetts, where same-sex marriage has been legal since 2004, ruled in July 2010 that the law is unconstitutional because it interferes with the right of a state to define the institution. A year later, 20 of the 24 bankruptcy judges based in Los Angeles ruled that the act violated the civil rights of a married gay couple who were denied the right to file a shared bankruptcy plan.

Last week, the Obama administration said it was extending its decision to stop defending the law to issues affecting actively serving military personnel and veterans in same-sex relationships.

In ordering the government to allow Golinski to enroll her wife in a family health plan, White rejected all of the arguments the House group advanced in defense of DOMA, such as that it was necessary to foster stable unions among men and women, and for Congress to act slowly on an issue on which the public remains divided.

White's decision "acknowledges that DOMA violates the Constitution and that my marriage to Amy is equal to those marriages of my heterosexual colleagues," Golinski said. "This decision is a huge step toward equality."

(AP) ? Twenty Republican presidential debates later, the head-scratching claims kept coming.

Did Mitt Romney really cut taxes as Massachusetts governor, as he asserted yet again? Or did he raise them by hundreds of millions of dollars, as former Pennsylvania Sen. Rick Santorum alleged? And how could Newt Gingrich have given the nation four balanced budgets when he was only in Congress for two of them?

There was something old, something new, in the misstatements of the candidates Wednesday in what was possibly the last GOP debate.

A look at some of the claims and how they compare with the facts:

ROMNEY to Santorum: "You voted to raise the debt ceiling five times without compensating cuts in spending."

THE FACTS: Maybe so, but increases in the debt ceiling were not politically charged in the past as they are now. They just allow the government to pay bills run up by previous Congresses. To not pay them would be like deciding to stop paying a car loan or mortgage. In fact, President Ronald Reagan, an icon to most conservatives, supported increases in the debt limit 12 times over his two terms. The idea of insisting on offsetting spending cuts when raising the debt ceiling is relatively new.

ROMNEY: "They finally realized I was right." ? On the government ushering the auto industry into and out of bankruptcy.

THE FACTS: Romney did propose a bankruptcy process for the automakers before the government opted for that course. But there was a tremendous difference between the course he advocated and the one that was taken. GM and Chrysler went into bankruptcy on the strength of a massive bailout that Romney opposed. Neither Republican President George W. Bush nor Democratic President Barack Obama believed the automakers would have survived without that backup from taxpayers. Romney held out the possibility at the time of the government giving certain loan and warranty guarantees that would not have approached the nearly $85 billion bailout.

SANTORUM: "Gov. Romney even today suggested raising taxes on the top 1 percent."

THE FACTS: Romney's new proposal actually would lower tax rates across the board. The rate for the wealthiest Americans would drop to 28 percent from 35 percent. However, Romney's call for unspecified new limits on tax deductions for higher-income taxpayers makes it impossible, absent more details, to assess the impact on any individual.

NEWT GINGRICH: "When I was speaker ... we balanced the budget for four consecutive years."

THE FACTS: Gingrich has made this misstatement many times before. He was speaker from January 1995 to January 1999. During budget years 1996 and 1997, when Gingrich was House speaker, the government ran deficits totaling nearly $130 billion. In budget year 1998, which ended Sept. 30, 1998, there was a surplus of $69 billion. And in budget year 1999, during which Gingrich was speaker part of the time, there was a surplus of $126 billion. Thus, Gingrich can only claim credit for contributing to two years of a balanced budget, at most.

SANTORUM: "Gov. Romney raised $700 million in taxes and fees in Massachusetts."

ROMNEY: "We cut taxes 19 times."

THE FACTS: Romney largely held the line on tax increases but the record is mixed. Massachusetts raised business taxes by $140 million with measures mostly recommended by Romney. As well, the Republican governor and Democratic lawmakers raised hundreds of millions of dollars from higher fees and fines, another form of taxation. Romney himself proposed raising nearly $60 million by creating 33 new fees and increasing 57 others. Anti-tax advocates praised his support for income tax cuts while objecting to his course on business taxes and fees.

SANTORUM, on Syrian President Bashar Assad : "If (Syria) would have been any other country, given what was going on and the mass murders that we're seeing there, this president would have quickly joined the international community, which is calling for his ouster and the stop of this. But he's not. He's not, because he's afraid to stand up to Iran."

THE FACTS: Obama, in a statement Aug. 18, 2011, said: "For the sake of the Syrian people, the time has come for President Assad to step aside." Since then, Obama and officials throughout his administration have repeated dozens of times their call for Assad to leave power immediately. The U.S. also led efforts at the United Nations to win support for a resolution demanding Assad's resignation. It was vetoed by China and Russia, but the Obama administration is now working with European and Arab partners to use diplomatic and economic pressure to force Assad out of power.

ROMNEY: "I'm going to link the pay of government workers with the pay in the private sector. Government servants shouldn't get more than the people who are paying taxes."

THE FACTS: It's something of a myth that federal workers make out like gangbusters next to their private sector counterparts.

The latest Congressional Budget Office study found federal pay is, on average, only 2 percent higher than for comparable private sector workers. The discrepancy is larger among the least educated. Federal employees with just a high school diploma make 21 percent more than similar private workers. But federal workers tend to be more educated, older and concentrated in professional occupations ? and they make 23 percent less, on average than private sector counterparts.

That advantage holds true when benefits are added to the mix: The federal professionals still lag, while federal employees with less education have a greater advantage over private sector workers.

GINGRICH: "If we're going to have a debate about who the extremist is on these issues, it is President Obama, who as a state senator voted to protect doctors who killed babies who survived the abortion."

THE FACTS: As an Illinois state senator, Obama voted against legislation promoted by anti-abortion activists that would have conferred protection to fetuses showing any signs of life after an abortion, even if doctors did not believe the fetus was viable. Obama pointed to an existing Illinois law requiring doctors to protect fetuses they believed were likely to survive after an abortion, and said he was concerned the proposed new law was so broad it could interfere with routine abortions. Obama said he would have supported federal legislation President George W. Bush signed in 2002 that would protect a viable fetus but reaffirmed a woman's right to an abortion.

GINGRICH: "It is utterly stupid to say that the United States government can't control the border. It is a failure of will. It's a failure of enforcement."

THE FACTS: A failure of will or enforcement is difficult to see in the statistics. Starting under the Bush administration, the ranks of the Border Patrol have risen to more than 21,400 agents, a force augmented by National Guard troops, unmanned aerial vehicles and fencing. A record 396,609 illegal immigrants were deported last year.

In the budget year that ended in September, border agents arrested the fewest number of illegal border crossers ? 327,577 ? in nearly four decades. That's considered a sign that fewer people are trying to cross, whether because doing so is riskier or because economic opportunity in the U.S. is less than before.

The debate presses on about whether the border is becoming secure enough, but there has been a measure of success and substantial effort.

Netflix bolsters video library as new threat looms

(AP) ? Netflix's Internet video service is adding more marquee attractions as it braces for a new competitive threat from cable-TV provider Comcast Corp.

As part of an effort to offer more exclusive material, Netflix locked up the right to show "The Artist" and other movies from The Weinstein Co. before the films are released to leading pay-TV channels such as Showtime and HBO.

Just a few hours after Tuesday's announcement about that multiyear deal, Comcast unveiled plans to undercut Netflix with a less expensive version of a service that will stream old TV series and movies to devices with high-speed Internet connections.

The Comcast service, called Xfinity Streampix, will be available this week to the company's subscribers. It will cost $5 per month, below Netflix's $8 monthly price for its Internet-streaming service. Some Comcast customers who pay for multiple services, including high-speed Internet access and phone, will get Streampix at no additional cost.

Although it appears Netflix's library is more extensive, Streampix could be good enough for some households looking to supplement their cable-TV service with a package that has on-demand video on computers and mobile devices.

That possibility apparently unnerved some investors who have long worried about Netflix losing its early lead in Internet video as more rivals enter the market. Netflix's stock lost $4.45, or 3.7 percent, to close Tuesday at $117.40.

Comcast, which is based in Philadelphia, will join two other large companies, Amazon.com Inc. and Wal-Mart Stores Inc., already offering video streaming services. Earlier this month, Verizon Communications Inc. announced that it is teaming up with Redbox's DVD rental-kiosk network to introduce an Internet video service later this year. The pricing for the Verizon-Redbox venture hasn't been disclosed.

Netflix, which is based in Los Gatos, Calif., also offers a DVD-by-mail rental service. But that has been losing millions of customers in recent months as the company has intensified its focus on the streaming service.

Netflix CEO Reed Hastings has said that he views cable TV as his company's biggest worry. So far, Hastings has identified Time Warner Inc.'s HBO channel as Netflix's toughest competition, but that could change if a cable provider such as Comcast can develop a compelling service.

By countering Netflix, Comcast hopes to hold on to customers, many of whom have been canceling their cable subscriptions to save money. Some of those former subscribers have been able to get their entertainment fixes from Netflix, whose Internet streaming service began this year with 21.7 million U.S. subscribers.

Comcast, by contrast, had 22.3 million video subscribers after losing 459,000 customers last year.

Based on the line-up included in Tuesday's announcement, Streampix will have much of the video already available on Netflix. The list of overlapping selections includes the past seasons of popular TV series such as "The Office" and "Lost" and older movies such as "Brokeback Mountain" and "The Big Lebowski."

Recently, Netflix has been trying to differentiate itself by buying the rights to more original series ? a strategy that has worked well for HBO and CBS Corp.'s Showtime.

The licensing agreement with Weinstein marks another step in Netflix's push to hone its competitive edge.

Getting the streaming rights to critically acclaimed fare such as "The Artist," which is nominated for 10 Academy Awards, also may help Netflix keep its subscribers happy as other popular selections disappear from the service's video library. Netflix will lose the rights to stream many movies from Walt Disney Co. and other studios when a three-year licensing deal with the Starz Entertainment channel expires next week.

The Weinstein agreement will give Netflix some movies within a year of their release in movie theaters. Besides "The Artist," the deal covers foreign-language movies, documentaries and other films in the Weinstein vault. Financial terms weren't disclosed.
